What is a Dental Bridge?

A dental bridge replaces one or more missing teeth in the mouth. They consist of an artificial tooth anchored to the neighbouring natural teeth.

The adjacent teeth need to be in good shape for implant supported bridges. If a tooth is lost due to trauma, gum disease or the lost tooth is too badly decayed to be saved, a bridge may be an ideal option to fill the gap.

Metal-Free Bridges

Metal-free dental bridges such as Zirconia bridges are made from high strength ceramic. They provide a natural colour match and are hypoallergenic. These bridges require less tooth structure trimming and they are a good option for front and back teeth.

Zirconia dental bridges come in two types:

  • Solid Zirconia - Suitable for back teeth and can withstand high stresses.
  • High Translucent Zirconia - Indicated for front teeth for a natural look.

At our clinic, we generally use metal-free bridges as they offer natural aesthetics, minimal tooth alteration without tooth decay, hypoallergenic properties, exceptional strength and versatility for a beautiful, long-lasting smile.

Metal-Containing Bridges

Metal-containing bridges consist of an underlying metal framework covered by a layer of tooth-coloured ceramic. The inner metal layer provides strength to the bridge, while the outer ceramic layer contributes to the aesthetics as its shade can be customised to match the remaining natural teeth.

These bridges can be used for both front and back teeth and are typically cost-effective. However, in some cases, the metal framework traditional bridge may become visible at the gumline, especially if the gums recede over time.

Types of Dental Bridges

Group 505-3-1
This is the most common type of dental bridge. It consists of artificial tooth/teeth being held in place by dental crowns that are cemented onto the neighbouring natural teeth on both sides of the bridge. Dental bridges have been shown to be long lasting and are also natural looking (with the right materials used). This method of replacing a missing tooth is popular as it does not require surgery and it is also fixed in the mouth, unlike removable dentures which can be a hassle.

They are similar to a traditional bridge but the artificial tooth is held in place on only one end of the bridge. Due to this design, cantilever bridges may not be as stable as compared to traditional bridges and is not recommended if you have strong chewing forces. It is also only possible if favorable conditions are present. The advantage of cantilever bridge is that only one natural tooth needs to be prepared to support the artificial tooth.
Group 505-5
They are also known as resin-bonded bridges. They use a framework of retainer wings instead of crowns to hold the artificial tooth in place. These wings are bonded to the backs of the natural supporting teeth to hold the bridge in place. The advantage of Maryland bridge is that the neighbouring teeth do not need to be drilled at all for this to be placed. The disadvantage is that the whole thing may feel bulkier due to the added material from the bridge on your teeth. Also, as there is no drilling, the bridge may need to be replaced in years when the cement bonding it to the teeth deteriorates. 
Group 505-6
This is similar to a traditional dental bridge but it rests on dental implants instead of neighboring natural teeth. Therefore, this type of bridge requires a longer treatment time, as dental implants must be placed and integrated into the bone first to support the dental bridge. The advantage is that it can replace multiple teeth with only two implants to support it, and also it is very strong and durable. No natural teeth need to be drilled for this as well.

Advantages and Disadvantages of Dental Bridges

Advantages

  • Restores a natural look to a person’s mouth/smile
  • Restores the ability to chew food efficiently
  • Prevents adjacent teeth from moving or tilting in the gap
  • Takes relatively less time to make compared to other alternatives
  • The process does not involve surgery
  • Can be placed immediately after tooth extraction

Disadvantages

  • Bridge preparation would require removing tooth structure from at least one of the adjacent healthy teeth
  • The supporting teeth on each side of the bridge may not be strong enough to support the bridge. If any of the teeth holding the bridge develops a problem, the bridge may need to be replaced or repaired
  • A risk of decay is present if the bridges over the supporting teeth are ill-fitting (thus allowing bacteria and plaque to form underneath the bridges)

Alternatives to Dental Bridges

Removable Partial Dentures

Although these do not require grinding down adjacent teeth, they are not nearly as stable or comfortable as dental bridges/implants and can affect speech and eating.

This type of restoration is less expensive but does not look as natural or function as traditional bridges as well. The bone underneath a removable partial denture may deteriorate over time, changing the appearance of your smile and face. Also, you will need to remove the partial dentures daily for cleansing and maintenance.

Dental Implants

Unlike a bridge, an implant stands on its support without putting strain on other teeth. This helps to protect surrounding teeth. Dental implants look, feel and function like a natural tooth. They are strong and stable.

Having an open space in your mouth where a missing tooth once was puts your jawbone at risk for deterioration. The teeth around the space also tend to shift and move into the space, becoming misaligned. Dental implants stimulate and preserve natural bone growth, which helps prevent bone loss and fills up the space from the missing tooth.

1. Dental Bridge Consultation

Our dentists will take photos and digital x-rays of your teeth and explain the treatment options available.

As bridges are usually irreversible, we need to make sure that consent is granted and that all alternative options have been considered. Our philosophy is for both our dentist and you to have a detailed understanding of the situation to make the right decision for yourself.

2. Tooth Preparation and 3D Scanning

To allow the bridge to fit well, we would need to remove a layer of tooth structure from the neighbouring teeth. This will ensure that the dental bridge is strong and fits well.

Once the tooth is adequately shaved down, we will use a 3D scanner (CEREC) to create a 3D model of your teeth, instead of traditional dental moulds which may be messy and uncomfortable. Thereafter, the exposed tooth will be covered with a temporary bridge.

3. Designing, Milling and Hand Finishing 

Using the CAD/CAM software, we will create the shape of the new bridge. The bridge will be designed and milled using quality materials to ensure an accurate fit with a precision fit of 25 µm to the neighboring teeth and optimum bite force.

The bridge will be milled from a single block of material (monolithic). The milled bridge will then be placed in a furnace that goes up to 1540°C for a predetermined amount of time. This process strengthens the bridge.

4. Cementing the Bridge

The dentist will check the colour and fit before bonding them into place. Once your approval has been obtained, the bridge will be cemented.

After the cement is placed between the bridge and your teeth, it is permanent and cannot be altered. Your bite will be checked, and the bridge will be adjusted and polished. Another set of photos or videos will be taken for record purposes.

5. Review Check

Your dentist will see you a week later for a review. He will check your bite, the stability of the bridge and ensure that you are comfortable with it – just like your own tooth.

I saw dr Justin for my implant which I did many years ago, the tooth was loose, and other drs told me to remove it as they could not find out what brand it was but dr Justin was able to take 3d xrays and locate the brand and specially ordered the part in. he saved me from having to do a surgery to remove and put a new implant which would have cost me a bomb. the tooth looks even better than the original thanks to the experieced dental technician Katsuya from Japan.

  3. Good Oral Hygiene: Maintain good oral hygiene by brushing and flossing your teeth twice a day and following any special instructions your dentist or hygienist has given you to keep your teeth and gums clean and healthy.
  4. Please notify us and come in for a check immediately once you encounter any problems after the treatment. Any delay may result in more damage to the condition.

These instructions will help you care for your dental bridge and maintain a healthy and functional smile.

  1. Local Anaesthetic: If you have been given an anesthetic, it will take a few hours to wear off so be mindful not to bite your lip or tongue. Wait after the numbness wears off before eating or drinking anything hot.
  2. Discomfort: Some initial discomfort is normal. If you experience persistent pain or discomfort, contact your dentist for evaluation.
  3. Bite Adjustment: If your bite feels uncomfortable after a week, inform your dentist.
